Physical exams are required for PreK, kindergarten, 6th grade and students entering Chicago Public Schools (CPS) for the first time.

🦷 Dental care

All Chicago Public Schools (CPS) students can receive dental care at no cost to families. Dentists visit Courtenay and provide exams and cleanings to students whose parents have completed and returned the consent form.

👓 Vision care

Chicago Public Schools (CPS) provides annual vision screenings for PreK, kindergarten, 2nd grade, 8th grade, new students and students with IEPs.

Students who need a full eye exam can receive an exam and glasses through CPS at no cost. A signed consent form is required.

🤝 Start with the school

Concerned about your child’s learning, communication, behavior, health, or participation at school? Start with their teacher, the school case manager, or the principal.

📝 Ask for an evaluation

Parents can request an IEP or 504 evaluation. Send a written request to the principal explaining your concerns.

If you cannot write the request yourself, the school must help you put it in writing.

An IEP provides individualized special-education goals and services. A 504 plan provides accommodations and other support so a student with a disability can access school.

Parents have the right to participate in evaluation and planning meetings.

🗣️ Information in your language

Translation and interpretation are free. Ask your child’s teacher or the school office if you need an interpreter.

📚 Support for English Learners

If another language is spoken at home, Chicago Public Schools (CPS) screens your child’s English proficiency. The results determine whether they receive English Learner services.

Si en su casa se habla otro idioma, las Escuelas Públicas de Chicago (CPS) evalúan el nivel de inglés de su hijo/a. Los resultados determinan si recibe servicios para estudiantes de inglés.

Services include English instruction and classroom support. Some programs also provide support in your child’s home language.
